Kaiser Permanente Bridge Program

 

C4 Atlanta is proud to partner with the Kaiser Foundation Health Plan of Georgia. Kaiser will offer an affordable health care plan to eligible C4 Atlanta Individual Members. The program is uniquely designed to help uninsured, income-eligible members obtain health care coverage for a low monthly charge for up to 24 months. Eligible members pay a fraction of the premium cost and Kaiser Permanente subsidizes the rest. Coverage includes preventive services, hospitalization, comprehensive pharmacy and more.

This program is intended as a benefit for eligible artists living in Kaiser Permanente’s 28-county metro Atlanta service area. Participating artists are expected to use the Bridge Program as an opportunity to grow as thriving, Atlanta-based artists. Kaiser is therefore offering this program only to individual members of C4 Atlanta. All applicants must attend an orientation session (usually scheduled the morning of monthly enrollment).

Eligibility Requirements

Please read the following requirements carefully:

  • The Applicant must be a current individual member of C4 Atlanta
  • The Applicant must complete a “member orientation” session offered by C4 Atlanta.
  • The Applicant and all applying dependents* must live in Kaiser Permanente’s metro Atlanta 28 county service area.
  • The annual aggregate household income of the Applicant and all dependents must be at or less than the current income guidelines (see below) for enrollment in the Kaiser Permanente Bridge Program.
  • The Applicant and all applying dependents cannot be eligible for or enrolled in any other type of health insurance program, including Medicaid, Medicare, PeachCare or employer-sponsored health care.
  • The Applicant and all applying dependents must be age 64 or under.
  • No Applicant and/or applying dependents shall have been previously enrolled in the Kaiser Permante Bridge Program.

* “Dependents” shall mean, the Subscriber’s spouse and/or child dependent(s) up to the age of 26 (ending at the end of the month of the 26th birthday).

Premiums are subject to change.

If you fail to send in your monthly payments, your coverage will be discontinued. Any medical care received after the cancellation date will be billed at the non-member rate dating back to the last payment.
