KP Bridge Program

The Kaiser Permanente Bridge Program is uniquely designed to help the uninsured and/or
low-income pay for a standard Kaiser Permanente for Individuals and Families (KPIF) plan.
The program will subsidize the full monthly premium for up to 12 months, with the potential to
renew for an additional 12 months*. Coverage includes preventive services, hospitalization,
comprehensive pharmacy, and more.

*Opportunity to renew for an additional 12 months is at the discretion of Kaiser Permanente.

This program is intended as a benefit for eligible artists living in Kaiser Permanente’s 28-county metro Atlanta service area. Participating artists are expected to use the Bridge Program as an opportunity to grow as thriving, Atlanta-based artists. Kaiser is therefore offering this program only to individual members of C4 Atlanta. RSVP link is at the bottom of this page. Please read the ENTIRE details before signing up for a spot.

The open enrollment period for coverage in 2015 has ended. If you need to enroll in 2015 coverage outside the open enrollment period, certain events may trigger a special enrollment period. For more information on triggering events and special enrollment periods, please visit the information page. To apply for the Kaiser Permanente Bridge Program during a special enrollment period, you must fulfill all eligibility requirements.
The open enrollment period for coverage in 2020 runs from November 1, 2019 until December 15, 2019. If you need to enroll for coverage in 2020,  please use the RSVP form below to attend an information session.

Eligibility Requirements–Please read the following requirements carefully!

C4 Atlanta

  • The Applicant must be a current individual member of C4 Atlanta
  • Eligible C4 Atlanta members must have completed one of the following classes:
    1. Ignite, entrepreneurship training seminar for artists
    2. AIM Atlanta, marketing workshop for artists
    3. Website Bootcamp, website building workshop for artists
    4. Hatch, 4-month training intensive for artists who wish to work with community

Participation in C4 Atlanta classes does not guarantee acceptance into the bridge program. C4 Atlanta is not an insurance provider. C4 Atlanta programs are designed to serve arts professionals.

Kaiser Permanente Bridge Program
  • All applicants, and applying dependents, must live in Kaiser Permanente’s metro Atlanta 20-county service area.
  • The annual household income for the applicant must be less than the current income guidelines for 100% of the Federal Poverty Level (FPL). See chart below for income household income threshold based on number of dependents.
  • The applicant and all applying dependents cannot be eligible for financial assistance through the Georgia Health Insurance Exchange; cannot be eligible for public programs such as Medicaid, Peach Care for Kids, or Medicare; and cannot have access to an affordable job-based health plan (plans exceeding 9.86% of the employee’s household income are considered not affordable).
  • The applicant must be age 64 or younger, and all child dependents must be younger than 26.
  • The applicant or applying dependent(s) cannot have been previously enrolled in the Kaiser Permanente Bridge Program.

* “Dependents” shall mean, the Subscriber’s spouse and/or child dependent(s) up to the age of 26 (ending at the end of the month of the 26th birthday).

Monthly Premiums

The Bridge Program will subsidize the KP GA Gold 500/20 plan or KP GA Signature Gold 500/20 plan.
The monthly premium is $0. Premiums are subject to change. Fees for some services will apply.
