KP Bridge Program

The Kaiser Permanente Bridge Program is uniquely designed to help those who are uninsured and low income pay for a standard Kaiser Permanente for Individuals and Families (KPIF) plan. As a Community Benefit initiative, the Kaiser Permanente Bridge Program will subsidize the full monthly premium for up to 24 months. Coverage includes preventive services, hospitalization, comprehensive pharmacy and more.

This program is intended as a benefit for eligible artists living in Kaiser Permanente’s 28-county metro Atlanta service area. Participating artists are expected to use the Bridge Program as an opportunity to grow as thriving, Atlanta-based artists. Kaiser is therefore offering this program only to individual members of C4 Atlanta.

The open enrollment period for coverage in 2015 has ended. If you need to enroll in 2015 coverage outside the open enrollment period, certain events may trigger a special enrollment period. For more information on triggering events and special enrollment periods, please visit the information page. To apply for the Kaiser Permanente Bridge Program during a special enrollment period, you must fulfill all eligibility requirements.
The open enrollment period for coverage in 2016 will begin on November 1, 2015. If you need to enroll for coverage in 2016, please use the RSVP form below to attend an information session. Upcoming info session dates:

  • Thursday, December 1, 2016 @ 12:00pm
  • Thursday, January 12 @ 12:00pm

Eligibility Requirements–Please read the following requirements carefully!

C4 Atlanta

  • The Applicant must be a current individual member of C4 Atlanta
  • Eligible C4 Atlanta members must have completed one of the following classes:
    1. Ignite, entrepreneurship training seminar for artists
    2. AIM Atlanta, marketing workshop for artists
    3. Website Bootcamp, website building workshop for artists

Participation in C4 Atlanta classes does not guarantee acceptance into the bridge program. C4 Atlanta is not an insurance provider. C4 Atlanta programs are designed to serve arts professionals.

Kaiser Permanente Bridge Program
  • The Applicant and all applying dependents* must live in Kaiser Permanente’s metro Atlanta 28 county service area.
  • The annual aggregate household income of the Applicant and all dependents must be less than the current income guidelines of 100% FPL (see below) for enrollment in the Kaiser Permanente Bridge Program.
  • The Applicant and all applying dependents cannot be eligible for or enrolled in any other type of health insurance program, including Medicaid, Medicare, PeachCare or employer-sponsored health care.
  • The Applicant and all applying dependents must be age 64 or under. A single subscriber must be age 19 or above.
  • No Applicant and/or applying dependents shall have been previously enrolled in the Kaiser Permante Bridge Program.

* “Dependents” shall mean, the Subscriber’s spouse and/or child dependent(s) up to the age of 26 (ending at the end of the month of the 26th birthday). Verification Requirements

Monthly Premiums The Kaiser Permanente Bridge Program will subsidize the KP GA Gold 0/20 plan. The monthly premium will be $0, if eligibility continues to be met. Premiums are subject to change. Fees for some services will apply.
